Hayden Panettiere‘s cause of death has officially been confirmed weeks after her tragic death at age 36.

Moments ago, the Greenville County Coroner’s Office released its findings more than a month after the ‘Heroes’ and ‘Nashville’ actress passed.

Her official cause of death was determined to be: accidental fentanyl and mixed-drug toxicity.

Officials have determined that Panettiere died from the toxic effects of fentanyl, 4-ANPP, alprazolam, methocarbamol, and quetiapine.

Her manner of death has been formally ruled an accident.

The confirmation follows earlier reports that investigators were examining whether a fentanyl-laced oxycodone pill played a role.

At the time, Panettiere’s rep urged the public to await the official toxicology findings rather than rely on unverified claims. While the ruling now confirms fentanyl and multiple other substances contributed to her death, officials have not publicly confirmed the origin of the drugs.

As previously reported, the actress was found unresponsive and in cardiac arrest at a residence in Greenville, South Carolina, on August 16. Emergency responders attempted to revive her, but she was pronounced dead at the scene.

An autopsy found no signs of trauma that contributed to her passing. The wider investigation into the circumstances surrounding her death remains ongoing.

Panettiere is survived by her daughter, Kaya, and her parents.
